Demonstrate your dedication and leadership in anesthesiology by becoming a Fellow of the American Society of Anesthesiologists (FASA). The FASA designation is ASA’s highest acknowledgment that recognizes years of dedication to exceptional education, leadership and commitment to the specialty. ASA Active members who meet specific qualifications and criteria are invited to apply for the FASA designation. Information Needed and Time to Complete
The application will take approximately 20 minutes to complete, including sending emails for letters of endorsement. You will need the following information to complete the application:
- Business Name and Address
- NPI number
- ABA or AOBA board and sub-board certification year and ID number
- Dates and details for all qualifications met and submitted on the application
- CV
- Bibliography (optional)
- Credit Card (Visa, Mastercard, American Express) - You'll be asked to submit payment at the end of the application process ($350 application fee)
Once the application is complete and 2 letters of endorsement from current ASA Active members have been received, the review and approval could take 4-8 weeks. Applications are reviewed and approved by the FASA Sub-Committee of the Committee on Membership. You can email us at FASA@asahq.org to check the status of your application.
